Referrals
Some insurance plans require a referral from your primary care physician prior to your visit in order for coverage. Specifically, all HMO plans require a valid referral. Please note that a physician’s order is not the same as a formal referral—we require an official referral submitted by your primary care provider and authorized by your insurance plan.
Please note that incomplete or incorrect referrals may delay scheduling. If a required referral is not available, patients may choose to proceed at the self-pay rate.
Valid referrals will include:

CPT Code
-
Referral start-end date
-
Number of authorized visits
-
Name of our provider you are scheduled to see
Completed referrals can be faxed to 713-965-4091 or emailed to info@cypain.com. Once your primary care provider has sent the referral, we recommend contacting our office to confirm it has been received. Patients are also encouraged to bring a printed copy to their visit.
